In consideration of the privileges granted to the Guest whose signature appears hereon, and as a condition precedent to the admission of the Guest to BASE, Guest (and Guest’s  parent or guardian if Guest is under the age of 18 years) agrees not to hold 100 Columbus Gym, LLC dba BASE liable for any damages or injury to persons or property arising from using,  attending or  participating in physical  exercise or from the use of it’s property of facilities or for loss of property as a result of theft. Guest (and Guest’s parent or guardian if Guest is under the age of 18 years) hereby assumes full responsibility and risk for any such injury or damages which occur to him or her and does hereby fully and forever release and  discharge BASE, its agents or employees from any and all claims, demand, rights of action or causes of action, present or future whether the same may be known or unknown, anticipated or unanticipated, resulting from or arising out of such use or intended use of said facilities and equipment thereof, including but without limitation, any claims of personal injury or property damage   resulting from or arising out of negligence of BASE, its agents or employees, or the negligence of any other persons present on the premises of BASE, its patrons or licensees, or from theft of property.  By signing front side, Guest  acknowledges that Guest has read, understood and accepted this waiver.
